Change of Practice Details
If you have:
- a new provider joining* your practice and would like to add this provider to your existing HICAPS facility - settling their HICAPS & EFTPOS payments to your bank account**
- a provider leaving your practice
- changed your practice bank account
Please Note: A request to change your bank account must be accompanied by a bank deposit slip or bank statement as confirmation of account details.
A request to add a new provider must be accompanied by a Medicare Australia or Medibank Private letter issuing the provider *number.
If you do not hold either a Medicare Australia or Medibank Private provider number and would like to transact via HICAPS as you are registered with other Health Funds for claiming purposes, please complete the 'Application to transact with HICAPS' form, accompanied with either a 'Provider Add/Amend Form' and fax to 1300 725 726.
